The paracetamol ceiling — 4,000mg daily at absolute maximum, 3,000mg as the safer working figure — almost always arrives first. Count every source, including cold remedies. When the paracetamol is what caps your dosing instead of the pain relief, that is the specific signal to move to a single-ingredient opioid, which has no such ceiling — not to take more tablets.

Spasm, Headache and Dependence Treatment

Not everything here is an analgesic. Soma and Carisoprodol are muscle relaxants that break the spasm-pain cycle keeping an injury painful. Fioricet and Butalbital target tension headache. Methadone, Buprenorphine and Suboxone treat opioid dependence as well as pain.

Tolerance, Dependence and Addiction Are Three Things

These get used interchangeably and mean quite different things. Tolerance is needing more for the same effect. Physical dependence is the body adapting so that stopping abruptly causes withdrawal. Both are expected in anyone taking an opioid regularly and neither is addiction — which is a behavioural condition, compulsive use continuing despite harm. Conflating them causes real damage in both directions: patients refusing relief they need, and others assuming that as withdrawal happened they must be addicted.

The Combinations That Actually Cause Harm

Opioid deaths overwhelmingly involve a second central nervous system depressant — alcohol, a benzodiazepine or a sleeping tablet — as their effects on breathing add together. The dose that was safe yesterday is not safe alongside two drinks tonight. Make sure every prescriber knows your full list. Constipation is the one side effect that on no account fades: start fluids, fibre and a stool softener from the first dose instead of waiting.
