Immediate Release or Extended Release

The second decision is duration. An immediate-release tablet lasts three to four hours and puts you in control of exactly when you are medicated — useful if you need morning cover but not evening, or if a twelve-hour tablet is costing you appetite at dinner and sleep at night. An extended-release tablet gives a full school or working day from one morning dose and removes the midday dose that children routinely miss and adults find awkward to take at work. Many people end up combining the two: an extended-release tablet for the baseline with a small immediate-release top-up for an afternoon that needs it.

Where Provigil Fits

Provigil (modafinil) sits in this assortment but is not a classical stimulant and is not a first-line ADHD treatment. It is a wakefulness-promoting agent licensed for narcolepsy, sleep apnoea and shift-work sleep disorder, and it works on the brain's sleep-wake switch instead of on the attention circuits. It is worth knowing about as daytime sleepiness and inattention look similar from the outside and are treated completely differently. If poor sleep is what is driving the difficulty, treating the night is the better route.

Finding the Dose

Stimulant dose is not calculated from age or body weight in any reliable way. It is found by starting low and titrating on observed response, and the first dose tried is rarely the final one. Bring specifics to the review instead of "better" or "not really working": what time the effect arrives, what time it fades, whether appetite or sleep changed, and which particular task got easier. Feeling wired, jittery or emotionally flattened means the dose is too high — not that the drug is wrong.

The routines that survive an ADHD brain are the ones that do not rely on remembering — keys on a hook, one calendar instead of four, tomorrow's first task written down before you stop working today.
